A pump creates a vacuum around the penis, which draws blood into the tissue. The immediate effect is a temporary increase in size and firmness. The honest framing of what they do is worth reading before you buy one.

What is temporary and what is not

The swelling is temporary. It lasts from minutes to a couple of hours and then subsides. Every pump does this, and it is the effect most people are actually buying.

Permanent enlargement is not established. Some users report gradual change over months of consistent use; the clinical evidence for it is weak. Buy a pump for the temporary effect and for the erection assistance, and treat anything more as unproven.

Erection assistance is the one medically recognised use. Vacuum erection devices are a genuine, doctor-prescribed treatment for erectile dysfunction, usually combined with a constriction ring to maintain the result. If ED is the reason you are here, that is worth raising with a doctor — the medical versions are fitted and monitored.

Air or water

Air pumps are the traditional design — a cylinder and a hand pump or bulb. Cheaper, and used dry. The Pump Worx Beginners and the Seven Creations Enlarger are the entry points.

Water pumps are used in the bath or shower. Water is incompressible, so the pressure distributes evenly around the tissue rather than concentrating — which is why they are generally considered gentler and why Bathmate built its range around it. The Hydromax 3 is the smallest; the HydroXtreme 11 is the largest.

Automatic pumps take the hand out of it. The Pump Addicted Rx11 and the Intense automatic control the vacuum electronically, which makes over-pumping harder.

Using one without hurting yourself

This is the part that matters, and the mistakes are all in the same direction.

  • Build pressure slowly and release the moment you feel pain, pinching or numbness.
  • Fifteen to twenty minutes maximum, in short sessions with breaks. Not one long one.
  • Stop if you see dark discolouration, pinpoint red spots or blisters. Those are burst capillaries, and they mean too much vacuum.
  • Never use a pump while on blood thinners, with a bleeding disorder, with Peyronie's disease, or with an untreated infection, without asking a doctor first.
  • A constriction ring goes on after, and comes off within thirty minutes. Never sleep in one.

More vacuum is not more result. It is just more damage.

Combination models

Several products here are a pump and a masturbator in one — the PDX Elite Suck N Pump and the Pump Addicted Rx7 among them. They are bought for sensation rather than for sizing.

Related: cock rings for maintaining firmness after pumping, and accessories for the penis for the rest of the shelf.

How long do the results last?

Minutes to a couple of hours. A constriction ring extends it, within the thirty-minute limit.

Air or water for a beginner?

Water, if the budget allows — pressure distributes more evenly and over-pumping is harder. Air is cheaper and works.

Does it address erection difficulties?

Vacuum devices are a recognised treatment, but see a doctor rather than self-treating. ED often has a cause worth finding.

Do I need lubricant?

For water pumps, no. For air pumps, a little around the base ring improves the seal — water-based, since most seals are silicone.
